FinalForms
FinalForms is the system the Covington Catholic uses to track physicals and health information. It is a requirement of every student-athlete (including youth sports) to have an up-to-date valid KHSAA physical on file through this system. We strongly encourage that each athlete’s sports physical be completed between May 1st and July 10th, to avoid having it expire during an athletic season.
Uploading Your Physical
If you are already registered on Final Forms, here is the link to upload the physical:
Once you have an account set up for you and your athlete, a complete physical, insurance information, parental consent, medical emergency information, etc. must be uploaded to your athlete’s folder. Please note we are required by the KHSAA to accept only the KHSAA Athletic Participation Form, Parental and Student Consent and Release, (Form GE04) and to have all 3 pages on file.
